1) In the name of the Father, and of the Son, and of the Holy Spirit. Amen.
2) Forgive me, Father, for I have sinned. It has been ____(say the amount of time)____since my last confession. These are my sins.
Tell the priest your sins.
3) After the priest gives you a penance to say or do, he will ask you to say an Act of Contrition.
Act of Contrition:
My God, I am sorry for my sins with all my heart. In choosing to do wrong and failing to do good, I have sinned against you whom I should love above all things. I firmly intend, with your help, to do penance, to sin no more, and to avoid whatever leads me to sin.
4)Listen while the priest gives you absolution:
God, the Father of mercies, through the death and resurrection of his Son has reconciled the world to Himself and sent the Holy Spirit among us for the forgiveness of sins; through the ministry of the Church may God give you pardon and peace, and I absolve you from your sins in the name of the Father, and of the Son, ( make the sign of the cross) and of the Holy Spirit. Why Confess sins to a priest instead of directly to God?

There are several reasons. First, because that is what Jesus wants us to do. On Easter Sunday night, our Lord told the Apostles, His first priests: "Receive the Holy Spirit. If you forgive men's sins, they are forgiven them; if you hold them bound, they are held bound" (John 20:22-23). Second, telling our sins to a priest teaches us humility, something we would not learn if we confessed our sins privately to God. Third, we receive graces from the sacrament of Penance. Fourth, by absolving us of our sins, the priest, who is representing Christ, gives us the assurance that our sins have been forgiven. And fifth, the priest can give us some sound advice on how to avoid sin in the future.
In his 1984 statement on Reconciliation and Penance, Pope John Paul II urged Catholics to confess their sins often to a priest, saying that it would be "foolish......to disregard the means of grace and salvation which the Lord has provided and....to claim to receive forgiveness while doing without the sacrament which was instituted by Christ precisely for forgiveness."

Act of Reparation to the Sacred Heart of Jesus
O Sweet Jesus, Whose overflowing charity for men is requited by so much forgetfulness, negligence, and contempt, behold us prostrate before Your altar,
eager to repair by a special act of homage the cruel indifference and injuries to which Your loving Heart is so frequently subject.
We are aware that we have had a share in such indignities, which we now deplore from the depths of our hearts. We humbly ask Your pardon and declare our readiness to atone not only for our own personal offences, but also for the sins of those, who, straying far from the path of salvation, refuse in their obstinate infidelity to follow You, their Shepherd and Leader, or, renouncing the vows of their baptism, have cast off the sweet yoke of Your law.
We are now resolved to expiate each and every deplorable outrage committed against You. We are determined to make amends for the manifold offences against Christian modesty, for the foul seductions that ensnare the innocent, for the frequent violation of Sundays and holy days, and for the blasphemies uttered against You and Your Saints. We wish also to make amends for the insults to which Your Vicar on earth and Your priests are subjected, for the desecration, by conscious neglect or terrible acts of sacrilege, of the very Sacrament of Your divine love; and finally for the public crimes of nations who resist the rights and the teaching authority of the Church which You have founded.
O divine Jesus, we now offer in reparation for these violations of Your divine honor, the satisfaction You once made to Your eternal Father on the cross and which You continue to renew daily on our altars. We offer it in union with the acts of atonement of Your Virgin Mother and all the Saints and of the pious faithful on earth; and we sincerely promise to make recompense, as far as we can with the help of Your grace, for all neglect of Your great love and for the sins we and others have committed in the past. Henceforth we will live a life of unwavering faith, of purity of conduct, of perfect observance of the precepts of the Gospel and especially a life of charity. We promise to the best of our power to prevent others from offending You and to bring as many as possible to follow You. Amen.
